Immunizations:

Daily Arrival Times:

  Students should not arrive before 9:00 A.M.

Medical Requirements:

  Medicine Administration Forms are required when a student needs medication during school hours.  Form for non-prescription medication must be filled out by a parent and witnessed in the office, as medicine is required.  Form for prescription medication must be filled out by physician and then filled out by a parent and witnessed in the office, as medicine is required.
